Patient forms

Effective September 2024, we will be charging a fee for the following services:

• A $25 fee:
  • Copies of medical records (excludes Dr. offices and medical facilities)
  • Special letters
• A $50 fee:
  • FMLA paperwork
  • Short-term disability paperwork
  • Disability accommodation paperwork
  • School accommodation forms

If you have not had an appointment with your provider within one month of requesting these services, or if you have not discussed your request with your provider, you may need to schedule an appointment to have the forms filled out.

Per our financial policy Genesis Behavioral Health has up to 15 business days to complete these requests. We apologize for any inconvenience this may cause. We understand that these services are important to our patients, and we are committed to providing them in a timely and efficient manner.
